Ozark Surgical Group is open Monday-Friday, 8 am – 5 pm. We are closed on major holidays.
Patients are seen by referral from another physician or by self-referral for a surgical problem. Appointments can be made by calling 870.425.9120.
- Form of payment (copay, co-insurance, and remaining deductible are collected at check-in)
- Driver's license or state (photo) ID
- Insurance card(s)
- List of medications

Ozark Surgical Group is a preferred provider for most insurance carriers including Blue Cross Blue Shield of Arkansas, Blue Card, Health Advantage, Medicare, and Arkansas Medicaid. Insurance benefits are filed for services provided by Ozark Surgical Group.
Patients who belong to an insurance group requiring a referral form from their Primary Physician are responsible for providing this to Ozark Surgical Group.
Co-pays, co-insurance, and remaining deductible are due at the time of service. Most insurance companies pay a portion of the surgical fee. Patients are responsible for the remainder of their balance. Our office will assist patients with questions or concerns. Visa & Mastercard are also accepted.
A parent or guardian must accompany and stay with any patient under the age of 18 for the duration of their appointment.
Please call our office during office hours with any questions concerning patient care. One of the nurses will return your call as soon as possible and will relay any concerns to your provider.
Our nurses are unable to answer medical questions from patients who have not yet been seen by one of our providers.
Evening and weekend surgical care is shared by all of the surgeons at Ozark Surgical Group. One of the surgeons will be available 24 hours each day for surgical emergencies. If you have a surgical problem after business hours please call our office at 870.425.9120. Our answering service will take your information and contact the on call surgeon. They will return your call as soon as possible.
New or established patients who must visit the Emergency Room are encouraged to ask for one of our providers if surgical care is recommended. One of our four surgeons is available to surgical patients 24/7.
Patients should contact our office as soon as they are aware they will require a medication refill. One of our nurses will refill prescriptions according to provider instruction as soon as possible. Prescriptions for pain medication may not be refilled over the phone and will not be refilled after hours or on weekends. No narcotics are kept at our office.
